Below, you will find helpful information related to my clinical practice.
Primarily, appointments are scheduled via referral from other medical and mental health professionals in the community. However, I do accept patients without a referral. To schedule an appointment, please call my office or use the submission form in the contact section of this website. If using the Internet, do not offer detailed personal information. Instead, leave your name, age, a short message about your concern, and a telephone number where I can most conveniently contact you.
If a scheduled appointment needs to be changed, please notify my office within 48 hours of the scheduled appointment. There is a charge for appointments missed or canceled with less than 48 hours of notice.
Emergency call is provided 24 hours a day. All clinical patients are provided my emergency pager number. If a life-threatening emergency arises, call 911 immediately.
Hours are by appointment. Typically, clinical patients are seen from the mid-morning through the early evening each weekday. Occasionally, weekend hours are offered. Every attempt is made to schedule convenient appointments for patients.
Prescriptions are not refilled over the phone or facsimile. All patients are provided ample medications to last until their next clinical appointment. The only exception where prescriptions are refilled over the phone is when a clinical appointment is missed due to an emergency. For these times, a follow-up appointment is scheduled as soon as possible and medications are provided for the interim period.
Following a patient's written request for release of information, records are sent. Billing for such services is a nominal charge that includes cost of reproduction and postage. Requests for psychiatric records from third parties are promptly ignored. All requests for psychiatric records to disclose information to third parties for the purposes of diagnosis, treatment, and payment require the patient's written permission by a mechanism explicitly outlined in California Civil Code Section 56.104.
Remittance is due at the time of service. I do not bill insurance. If you have health insurance via a PPO, I provide a superbill that you can submit for reimbursement. For patients covered by a HMO, reimbursement can be acquired via submission of a superbill if prior authorization is obtained for out of network psychiatric care.
